How to process DIS request ?

Delivery Instruction Slip (DIS) Request Process:

The DIS slip is essentially like a cheque but for your Demat account transactions. It is used to facilitate and authorize the sale or transfer of shares from one account to another. In order to conduct a transaction, you are required to submit a DIS slip to your Demat account provider or DP (depository participant). After completing the authorization step the share will be transferred from your Demat account to the buyer demat account.

 

We need to follow the mentioned procedure for DIS Booklet request. 

In Zerodha: 

Download the DIS request form fill all the required details and courier it to Zerodha HO. Zerodha Head Office#153/154 4th Cross Dollars Colony, Opp. Clarence Public School, J.P Nagar 4th Phase, Bangalore – 560078. Within 7-10 days the DIS -Booklet will be sent on your registered address.

HDFC Securities:

To raise DIS request in HDFC Securities one has to first log in to your net banking account and follow the demat section. Now you will get the option of DIB Booklet, just click on the option and submit the details and continue. Once the request is accepted by HDFC Securities the DIS will be delivered to your registered address within 7-10 days. 

Logon to net banking>Demat>DIB Booklet>Continue.

 

ICICI Direct: 

“Delivery Instruction Slip (the DIS) booklet can be requested by visiting the nearest ICICIBank Demat Servicing Branch. You may also request DIS Booklet for your Demat account by following path: 

Log into your ICICI bank through internet banking >Investment and insurance>Demat>Service request>Request for DIS Booklet.

Or

You can raise the request on mail by mention your DP Id and cliemt id – customercare@icicibank.com. Or call on toll free no. the DIS BOOKLET will be sent by the bank to your registered address.

 

Groww:

You can raise your DIS Booklet request via ticket or chat. Groww will share the DIS request slip to your email address. Download the slip, fill in all the details and courier to GROWW HO. #11,80 Feet Road, 4th Block, St Bed, Koramangala, Bangalore, Karnataka 560034. The Groww will send DIS -Booklet on your registered address within 7-10 days.

 

Angelone:

In Angelone you can raise your request from your registered e-mail id at mail id disrequest@angelbroking.com , need to mention Demat BO ID (DP & Client ID) in subject or mail body.

 

Upstocks:

In Upstocks you require DIS Booklet then kindly place DIS request the request form attahed .

Kotak Securities:

You need to login to your Kotak Securities Portal web or application now click on More option and follow the service. Then click in Support Service Request - Follow Demat Account Now you will get the option of DIS Slip. You can raise your DIS request through this option. DIS will be delivered to your registered address within 5-7 working days.

 Logon>MORE>Service>support service request>Demat Account>DIS Slip.

 

5Paisa. 

In 5Paisa Demat account, For DIS Booklet you can request an E-mail Id support@5paisa.com  by your registered mail. The DIS request form will be provided by the 5Paisa on your mail id just need to fill the form and courier to the 5Paisa office. Once the courier receives by the 5Paisa office, it will take 10-15 days to deliver the DIS-Booklet to your registered address.
